Conversion from gas to hypochlorination or vice versa (PWS serves at least 350 persons or 150
connections or greater than 10 lb Cl/day).Water distribution system when more than one clearance is required

Technical Information required for new/modified water plants:
-SWFWMD Well Construction Permit and Consumptive Use Permit (if applicable)
-Well driller’s completion report and final grouting report approved by SWFWMD
-Results of the well pumping test (well yield, drawdown, static water level, etc.)
Chemical analysis of the raw water for the following parameters:

-Documentation and specifications of treatment chemicals/coatings
-Design average day, maximum day, and max. hour demands
-Plan & profile views of the overall plant layout, control equipment and process instrumentation &
control diagram
-Information required for Distribution System
-Each sheet of plans originally signed and sealed by Engineer of Record
-Approved pipe materials: NSF, AWWA C900, etc.
-Pipe sizes adequate (for hydrants, etc.)
-Adequate cover over water lines: typically 30-36"
-Adequate valves provided
-Adequate blocking and packing provided
-Blowoff’s provided
-Air relief valves provided for high points
-Sample points identified
-Proper backflow devices installed
-Sanitary hazards identified and avoided
-Horizontal and vertical separation distances between water mains and storm, sanitary, reclaimed, and
force mains are maintained (Profiles at crossings are shown)
-Method of interconnecting to existing water lines shown
-Service connections are identified
GENERAL PERMIT APPLICATION
This type of application is appropriate for the following projects:
-Water main extension;
-Replacement of existing lines using a different trench;
-Construction utilizing alternative materials;
-Construction of an extension of any length and any diameter
A general permit would not be allowed for projects that:
-have dry lines
-involve applicants with a history of MCL
or water quality compliance problems
-involve multiple non-contiguous projects (a separate permit would be required for each non-contiguous project)
-involve systems without sufficient reserve capacity
-include treatment, pumping or storage facilities or conflict manholes
-have water mains installed in areas contaminated by low-molecular-weight petroleum products or organic solvents
-interconnect previously separate PWSs or create a new PWS

Permitted construction or alteration of a public water system may not be placed into service until a
letter of clearance has been issued by this Department.
