Florida Geological Survey Forms

 

The Florida Department of Environmental protection has implemented a paperless form submittal process that will allow for the electronic submission of sinkhole reports and other forms.  In order to complete the following DEP electronic forms, you must have Adobe Reader 8.0 or higher.  We suggest that you download Adobe Reader 11, which is free, from the Adobe website at:  http://get.adobe.com/reader/

You may fill in the forms, save them to a locatable folder on your computer, and submit them to the FGS as an email attachment.  The "submit" buttons on each form provide the appropriate addresses, and in most cases will automatically open your email service with the send-to address pre-filled. Use the "Attach File" function in your email to attach the filled form that was saved on your computer.
 
  • FGS Publications Order Form - Use this form to order in-print FGS publications (Note: Payments must still be mailed via postal service mail).
  •   Orders are shipped once payment is received.

  • FGS Sinkhole Report Form - Use to report and describe new sinkhole occurrences in Florida. Data will be added to the FGS SIRs database.


  • FGS Sampling Proposal Guidelines and Proposal Form - This form describes the FGS sampling guidelines and requirements and includes the proposal form on which researchers may submit their sampling request.


  • FGS Well Sample Form - Used by staff, researchers, and field crews to document new geological samples delivered to the FGS.
